Tournament Registration Form

Choose from the list below, the tournament (s) you wish to enter.  Fill in your Name and Contact information, and hit the “Submit” button.  Prices are listed for your information only.  All tournament fees are paid at the door, not from the web site. 

             - Show up at least 15 minutes prior to the tournament start time to ensure placement                in the tournament.

             - No-Shows will be replaced with standbys.

                 - If you do not meet the minimum age requirement according to the ESRB Rating System and                    we do not have a Parental Consent Form on file for you then you must download, fill out,                    have parent/guardian sign, and bring it in with you at tournament time.  If you are unsure                    then follow this link to Game Ratings & Descriptor Guide to find out if you need to have                    a Parental Consent Form signed.  Click on this link to download a Parental Consent Form or                    go to the “Forms” tab and click on “Parental Consent  Form”.
